Please be sure to include the camper’s name on the check. Camper is not registered until check has been received.

We/I hereby request that you accept this application and in consideration of your acceptance of the application, we/I hereby release 杏吧原创 and all its employees from all claims on account of any injuries which may be sustained by our/my child while attending the 2017 杏吧原创 camps. We/I shall indemnity and hold harmless 杏吧原创 and all of its employees and against any loss or expense with respect to any action, claim or demand by we/I and my/our child for injuries sustained by our/my child. Furthermore, we/I certify that within the past year, our/my child has had a physical examination and that he/she is physically able to participate in all activities.
